Returning to full strength after an ACL injury is possible with the right guidance. Physical therapy plays a central role in ACL surgery recovery, helping patients regain mobility, rebuild strength, and safely return to their favorite activities or sports. At Aspen Rehab, we provide a structured, personalized plan that supports every phase of recovery and helps reduce the risk of re-injury.
This guide outlines the essential steps of a successful ACL recovery process, including what to expect, how to progress, and how physical therapy makes each stage more effective.
Starting Strong: The First Few Weeks After Surgery
The early stage of ACL surgery recovery focuses on protecting the surgical site, managing swelling, and beginning gentle movements. A movement specialist at Aspen Rehab will help guide you through:
Swelling and Pain Control
- Elevation, compression, and targeted exercises to reduce inflammation
Range of Motion Exercises
- Gentle knee bending and straightening to prevent stiffness
Muscle Activation
- Isometric exercises to engage the quadriceps and hamstrings safely
Walking Mechanics
- Gait training to help you move without compensations or limping
The goal here is to lay the groundwork for safe progression while keeping the knee stable and supported.
Building Strength and Stability During ACL Surgery Recovery
As pain decreases and mobility improves, the next step in ACL surgery recovery involves strength building and coordination. This stage includes:
Progressive Resistance Training
- Adding resistance with bands, weights, or body weight to target key muscles
Balance and Proprioception Drills
- Exercises on unstable surfaces to retrain the body’s joint awareness
Core and Hip Strengthening
- Stabilizing the pelvis and trunk to reduce stress on the knee
Cardio Reintroduction
- Low-impact conditioning to improve endurance without joint strain
This phase is crucial in restoring symmetrical strength between both legs and preparing for dynamic movements.
Back on Your Feet: Returning to Activities and Sports
Once foundational strength is restored, your physical therapist will guide you through sport-specific or activity-specific retraining. This return-to-play phase includes:
Jump and Landing Mechanics
- Practicing soft, controlled landings to protect the knee
Agility Training
- Side shuffling, directional changes, and reaction-based drills
Running Progression
- Gradual reintroduction of jogging, sprinting, and cutting patterns
Functional Assessments
- Tests to measure strength, control, and symmetry before a full return to sport
At Aspen Rehab, your therapist will only advance you when objective criteria are met to ensure safety and prevent setbacks.
Long-Term Success: Preventing Future ACL Injuries
Finishing therapy does not mean the work is over. Prevention remains an ongoing priority, especially for athletes. Key strategies we emphasize include:
- Continuing strength training, particularly for the hamstrings and hips
- Maintaining neuromuscular control through regular dynamic warmups
- Wearing appropriate footwear for sport or terrain
- Staying consistent with technique drills to reinforce proper movement mechanics
- Performing regular check-ins with your physical therapist if returning to high-level sports
These strategies help reduce the chance of re-injury and keep you active for the long run.
